TD exceptions investigation

These are persisted TD observations which are either unmatched (no SMART factors were found) or genuinely ambiguous (competing SMART offsets for the same STANOX, direction and arrival/departure role). Factors at different locations are sequential evidence and are all applied. For an ambiguity, red identifies the deterministically selected factor; it does not establish that factor as railway-correct.
